Fitness Membership Reimbursement

Single Membership:
GNA will reimburse up to a maximum of $20 per month per employee

Single Membership Example:
Membership Fee = $15 – GNA Reimburses $15

Family Membership:
GNA will reimburse up to a maximum of $40 per month per household

Family Membership Example:
Membership Fee = $60 – GNA Reimburses $40

Online Membership:
GNA will reimburse up to a maximum of $20 per month per household

Online Membership Example:
Membership Fee = $30 – GNA Reimburses $20

Note:
Reimbursements may now be submitted on a monthly or quarterly basis and must be submitted within 30 days of the end of the quarter or they may not be accepted.
